Water Quality Questionnaire

Our Goal: Providing Quality Drinking Water

Even though SGWASA meets all State and Federal drinking water standards, sometimes utility customers experience water quality issues at their home or business due to a variety of issues.  To assist SGWASA in better understanding the water quality issues customers are experiencing, SGWASA created a Water Quality Questionnaire.  

Please use the following questionnaire to tell us about a water quality concern that occurred within the past 2 days. The information you provide to us helps us to better understand, and document what may be occurring at your SGWASA service address.  At the end of the Water Quality Questionnaire, you will be asked: What action would you like SGWASA to take? You will be able to select one of the the following answers: 

  • No further action, I just wanted SGWASA to know about my water quality.
  • Please consider flushing the water system near my address.
  • I would like SGWASA to flush the area near my address and then contact me to discuss my in-home water quality concerns.


The completed Water Quality Questionnaire will be reviewed by key management staff to determine what follow up actions need to occur based on the information provided.
